Build the application form
Three fields are always there. Everything else you add from the standard list, from a template, or as a custom field you write yourself.
Every plan has its own application form, and Application Form Fields is where you decide what you ask. Kannect's brief for the section: "Define the information you want to collect from applicants. Build a custom application below, or start from a saved template and add more."
What is already there
Three fields are on every form and cannot be removed:
- First Name
- Last Name
- Email address
Everything below them is yours to choose. The list shows each field's name on the left and a Required toggle on the right, and under a custom field it shows you what you set it to, for example "Data type: number, Min value: 0, Max value: 10".
Steps
- On the plan form, scroll to Application Form Fields.
- Optionally pick a template from Start from a template and choose Add Fields.
- Choose Add Field to add one at a time.
- Set the Required toggle on each field you insist on.
- Choose Save Membership at the bottom.
Start from a template
The dropdown lists your saved application templates with their field count, for example "Donor Template 1 (4 fields)", plus Custom application (start blank).
Choosing Add Fields does what the note under it says: "Adds the template's fields to the form below. You can still add or remove fields after." On an existing plan the note is even clearer, "existing fields are kept", so adding a template tops up the form rather than replacing it.
Templates are the right move once you have more than two or three plans, because they keep your questions consistent and save you rebuilding the same form.
Add one field at a time
Choose Add Field to open the Add Application Field panel. There are two ways through it.
Pick an existing field
Open Select Field and choose from:
- Standard fields: Date of birth, Address, Country / State / City / Postal Code, Phone Number, Gender, Marital Status, Interested in Volunteering?, Emergency Contact Name, Emergency Contact Number.
- Every custom field you have already created, anywhere in your account, each shown with its data type in brackets.
That second group is the useful part. A custom field you wrote for an event ticket or another plan is available here, so you ask the same question the same way everywhere and the answers line up when you export.
Create a new custom field
Choose + Create a new custom field at the top of the same list, then fill in:
- Custom Field Name, the question as the applicant reads it. The placeholder is "e.g., Dietary Requirements".
- Data Type, one of Short Text, Long Text, Number, Date, Checkbox or Radio.
Then set Is this field required? to Yes or No and choose Save Field.
